Dental Insurance
We accept most major insurances and PPO insurances at our office, including the following:

As a courtesy, Zubkov Dental will submit your claims to your insurance company for you!
Our office utilizes an electronic claims processing system. Rather than sending your insurance claim through regular mail, our team sends it to your insurance company electronically. Electronic claim submissions are typically more efficient, contain fewer errors, and are delivered much faster.
We Accept All Major Credit Cards
No insurance? No problem! Zubkov Dental currently accepts cash payments, checks, and all major credit cards. For patients who may need extra support in financing their dental treatments, we offer third-party financing through CareCredit®, as well as in-house payment plans to those who qualify!

Smile Club Terms & Conditions
All membership fees are non-refundable and due in full at the time of enrollment. Your benefit year will run a full calendar year (i.e. July 1st through June 29th). Because the Smile Club entitles members to a generous discount of 20%, we also require that all treatments be paid for in full when the appointment is made.
Some patients may require additional x-rays for proper diagnosis. If needed, small additional charges may apply.

Dental Savings Plan
At Zubkov Dental, we never want a lack of dental insurance to keep you from the healthy smile you deserve! To make treatment more accessible for all patients, we’ve created the Smile Club, a private, in-house savings plan that allows you to bypass third-party financing and afford quality dental treatment. Smile Club benefits include significant discounts on dental cleanings, preventive exams, digital x-rays throughout the year, and even select dental procedures!
Members of our Smile Club can enjoy the following benefits: NO deductibles • No maximums • No waiting periods
The team at Zubkov Dental is proud to offer this exclusive dental discount program to patients without dental insurance. The Smile Club membership is a private dental savings plan, valid only at our office — it is NOT an insurance plan! We have a variety of membership plans to serve individuals, families, patients focusing on preventive care, and patients fighting periodontal disease.
Preventive Program
As Smile Club Member, your preventive care will include:
- 2 thorough brighter, whiter dental cleanings in a 12-month period
- 2 doctor examinations in a 12-month period
- 1 set of diagnostic cavity-detecting x-rays in a 12-month period
- 2 mineral treatments used to help support healthy tooth enamel
- 1 emergency visit with x-ray in a 12-month period
- An additional 20% discount on other dental services, should you require any treatment. Exclusions may apply for some cosmetic treatments and surgeries.
How Much Does a Smile Club Membership Cost?
Your total dental savings plan cost will depend on your age and necessary treatments. Smile Club membership options include the following:
$384 annual enrollment fee for adults aged 12 and older
$329 annual enrollment fee for children under 12
Dental Savings for Periodontal Treatment
Patients with periodontal disease will typically require more frequent, thorough cleanings each year. For these patients, Zubkov Dental is proud to provide affordable options to promote optimal oral health and maintenance. Our annual enrollment fee of $545 for periodontal patients includes 3 periodontal cleanings per year. Patients will also be eligible for 20% off scaling and root planing! Patients must have their periodontal disease under control prior to eligibility.
